Work permit applications are processed, filed, and issued at ROE 30. To make an appointment, please call one of our offices to assist you in completing the process. 

Please, bring the following to your appointment:

• Complete Work Permit Application

  • Leave all signature lines unsigned, until a member of our staff can witness

• Birth Certificate

• Social Security Card 

• Health Physical (dated within one year of the date you are applying for work permit)

• Letter of intent to employ from prospective employer, dated within one year of application.

• Verification of satisfactory academic progress to work part-time 

  • Only required during school year (September 1- June 1)

• Parent or Guardian State ID or Drivers License 

  • Parent or Guardian MUST be present at the time the application is filed under Section 205/12 of the Illinois Child Labor Law.
